I am Kanto, Johto can get the passport making the quest or not?

Not sure what you mean, if you started in Kanto you can go to Johto if you want as long as you complete all the Kanto Gyms first.

"Johto passport", is only for those who get in Johto?
Or who started in Kanto can get it too?

The passport decides which region you go to first. So if you start in Kanto, you get the Kanto passport. The passport has no further use. Once you collected all 8 badges in Kanto, you will be able to go to Jotho and beat the gym leaders there as well.
You won't get a Jotho Passport item, but it is only there for the game to decide to which region to spawn you when you choose your starter Pokemon. So doN't worry, you will have access to Jotho as well

On top of what Merse said, the only other places that do require passports are the starter quests in the beginning of each region. Hidden Crag/Pokeball Quest, you can only do one of these depending on your passport.
